Payment is made via Credit or Debit card. If you choose recurring payments, you will be billed monthly on the same day, each month that you made your initial payment for 4 months. If you choose to pay month to month your payment will be made each month, and you will be responsible for reordering. After the first cycle is finished you will begin the second cycle. You will at that time enroll into the second cycle with a subsequent blood exam (no cost to you). A new form will be sent via email requesting you to choose recurring payment. or month to month.  This will continue until you have reached your weight loss goals, and discontinue the program, or 72 weeks at which time we will need to reevaluate your condition.

First Cycle is 16 weeks. If you decide to have recurring payments it will be $400 per month for the first cycle (16 weeks) of the Integrity Medical Weight Loss Program.  If you decide to choose month to month the payment is $425 per month for four months.  Again, when paying month to month you are responsible for reordering.

Second Cycle is weeks 17 to 72.  If you decide to have recurring payment it will be $475 per month for four months. If you decide to choose month to month the payment is $500 per month for four months.  Again, when paying month to month you are responsible for reordering. After each 4-month period you will again be asked to take a subsequent blood exam (no cost to you). You will then be sent an email requesting you to choose either recurring payment or month to month.

You will be reevaluated every 4 months during the course of the second cycle. Until you have met your individual goal or 72 weeks has elapsed.
